At Hope and Life, we understand that every participant's journey is unique. Our team are dedicated to providing the necessary support and guidance as you navigate through significant life stages and transitions.
The NDIS Assist-Life Stage, Transition program is specifically designed to support participants during different stages of their lives, and to promote independence and social inclusion. We acknowledge the specific needs and aspirations of each individual, and nurture the development of necessary skills to live independently and actively participate in the community.
We are here to help you with planning for change between phases of life, and developing strategies to cope with the challenges that can arise during transition periods. Our approach is highly individualised, taking into consideration your abilities and specific requirements. Our support for life stage transitions includes both short-term and long-term assistance.

Nurturing your journey through the transitions and significant events of life towards a brighter future by assisting with:

Support connection and coordination (including linking to and organising a network of supports and services)

Capacity building skills (e.g. managing household duties, managing finances, daily planning, decision-making)

Community engagement and social inclusion (e.g. participation in events, building positive relationships, integrating into society)

Education and employment support (e.g. enrolling in education or career programs, job seeking)

Housing and accommodation support (e.g. seeking for suitable accommodation, adjusting from a shared space to independent living)

Adapting to major life changes (e.g. graduating from school, marriage, parenthood, illness, overcoming loss of a loved one)
